Youthful Aging Service
Kobernick-Anchin, in partnership with Youthful Aging Home Health, has established Youthful Aging Service, a progressive health and wellness model for independent living communities.
A premier independent retirement and assisted living community established in Sarasota more than 15 years ago, Kobernick-Anchin enjoys an excellent reputation for the high quality of its independent living and assisted living at its Kobernick House and Anchin Pavilion, respectively.
In 2008, The Kobernick House launched a more progressive model of health and wellness services for its aging independent retirement community. As residents age, remaining functionally independent while living with chronic health conditions becomes a priority. The new model, developed in partnership with Youthful Aging Home Health, enables residents to remain in their homes for as long as possible.
As residents age and move along the continuum of needed services, health care support increases. A personalized fitness and wellness program wraps around the health services continuum. Health education becomes a major focus. Each resident has a personal session with our nurse to become educated about their medical conditions and how to prevent complications.
The primary concern for this aging population is preserving independence. The Kobernick House ensures a safe and secure environment while the Youthful Aging Service continuously works to preserve functional independence.
With this emphasis, residents will enjoy a greater opportunity to maintain their independence and improve the quality of their life.
Youthful Aging Service supports residents in maintaining their maximum physical and memory capacity, and in slowing the loss of independence for as long as possible. Three services are fully integrated:

Comprehensive management of chronic conditions by our full-time nurse who focuses on prevention and early intervention of clinical issues that could lead to complications.

The electronic medical record system keeps nurses and physicians informed at all times thus reducing the potential for more serious medical errors.

An individualized physical fitness and rehabilitation service, including memory enhancement programs.

Youthful Aging Home Health Services
Youthful Aging Home Health provides the following services to support Youthful Aging Service at The Kobernick House.

Youthful Aging delivers client centered care to seniors living in Sarasota and Bradenton Florida. Our Registered Nurses work with Certified Nursing Assistants (CNA's). Youthful Aging Companions deliver proactive wellness services in addition to meeting basic senior citizen needs. Youthful Aging's progressive approach to home health care enables greater independence as long as possible.
