… and out of nursing facilities … What you can learn to help your own
patients
Sometimes, home health agencies lose patients when they get well, but just as often they lose patients who move along the continuum to nursing facilities. But one HHA has discovered a way to help their patients live almost independently.
Longevity is a division of Youthful Aging Home Health, and is designed to advise older adults on options for continuing to live independently in their own home. Improving memory and physical mobility become the keys to sustaining independence.
“Longevity accomplishes this goal through our six-point formula for health management and wellness. It reduces the need to relocate into the retirement continuum from home to independent living to assisted living and then to a nursing facility,” says Nicci Kobritz, RN, NP, president of Youthful Aging Home Health Care in Sarasota, FL.
Clients pay only for what they need so they can maintain flexibility, choice and control. Longevity’s target market includes the older adult who:
• Chooses to continue retirement at home
• Is experiencing complex health conditions
Our role is to provide quality senior care with options. It’s the ultimate in supporting independence. Call us today to learn more: 941-925-9532.

The Longevity Service
Services are personally delivered by a RN in partnership with each resident’s physician and accessible 24 hours a day. This personalized system is designed to maintain and promote greater physical mobility, memory acuity and social independence. This is accomplished by teaching clients to self-manage their health conditions while integrating the latest prevention and wellness technology.
The 6-Point Formula
• Stabilize Condition: Partner with treating physicians to ensure chronic health conditions remain stable, including changes in medications. Teach self-management of health conditions, including annual prevention screens and immunization. Emphasis should be placed on evaluating early signs of memory loss and related dementias.
• Physical Fitness and Rehabilitation: Affiliate with premier physical therapy and personal training companies. Aim exercise at strengthening muscles; endurance; gait and balance training. Physical therapy; speech therapy; occupational therapy; art therapy; and massage therapy should also be incorporated.
• Brain Fitness: This is aimed at preserving and improving memory to develop stimulating brain exercises through brain simulation exercise software programs that have demonstrated improvements in memory.
• Nutrition: Special diets and education about the importance of nutrition and hydration.
• Socialization: Encourage engaging in activities. Life-long learning programs should be designed to respond to all levels of cognitive and intellectual functioning, both within the home and within the community.
• Smart Home Technology: Greater independence in the home may be achieved through a variety of assistive devices. For example:
• Home security features including remote monitoring
• Telecommunications devices and video equipment
• Assisting devices which support daily living activities: bathing; dressing; medication reminders
• Emergency Response System (necklace and bracelet)
• GP sensors ad motion devices to monitor nighttime activity
